Information for referrers
Sage Nutrition is a Brisbane-based private dietetic practice providing evidence-based nutrition care for patients with eating disorders, disordered eating, and gastrointestinal conditions. I work collaboratively with GPs and multidisciplinary teams, with a strong focus on patient safety, autonomy and sustainable change.
Who Can Refer
Referrals are accepted from:
General Practitioners
Psychologists and psychiatrists
Allied health professionals
Specialists
Hospitals and health services
Self referrals are also accepted.
While a referral is not required to book an appointment, a GP referral is recommended for patients wishing to access Medicare rebates (Eating Disorder Treatment Plans or Chronic Disease Management Plans).
Communication
With patient consent, I provide assessment summaries and progress updates, and will promptly communicate any concerns requiring medical review or escalation of care. If you have specific preferences around reporting or frequency of updates, I’m very open to discussing this.

Dietary interventions are approached with a strong emphasis on minimising restriction where possible.
How to refer
Referrals can be made via email - please send a referral letter to shannon@sagenutrition.com.au.
Please include:
Reason for referral
Relevant medical history and medications
Recent pathology and weight history (if clinically relevant and available)
Any current care team members
Once I receive your referral letter, I will contact the patient to arrange a consultation. Alternatively, patients can book directly via the online booking system.
Fees
Initial appointment (60 minutes) - $180.
Review appointment (45 minutes) - $100.
